Need a Duplicate Car Title in Illinois?

The registered owner of the vehicle should do the following:

  1. Fill out the Illinois Application for Vehicle Transactions (VSD190) online form. Check the box that says “Duplicate title”
  2. You have the following options:

    Bring your duplicate title application to your local DMV office.

    or

    Mail your application to the Illinois division of motor vehicles at:

    Secretary of State
Vehicle Services Department
ERT Section, Rm. 424 (use Rm. 434 if expedited processing is needed)

Please note that there may be a waiting period before a duplicate title is issued and you will need to make out a check for $95.00 to the Illinois DMV.
